As a Nurse Practitioner, you have a wealth of knowledge and experience. Now it's time to open your world--and heart--to even greater, richer experiences with Evercare, a UnitedHealth Group company. We're the largest single care coordination program for aging, vulnerable and chronically ill individuals in the nation. Our Nurse Practitioners are personally involved in the care of their patients. New expansion into the Albany, NY market has created a need for us to hire a new Nurse Practitioner (NP). The position offers independence and excellent benefits from a Fortune 100 healthcare leader. The hired NP will be doing direct care among a geriatric population.

Key responsibilities include:
Managing a patient load of about 80 members throughout the Albany region
Primarily working out of one or two facilities
Working closely with primary care physicians and nursing staff, performing physicals and geriatric assessments, prescribing medications and ordering lab results
Acting as primary contact with the facility

Qualifications
Only licensed NPs certified in geriatrics, adult or family practice will be considered for this position. Clinical experience gained as an NP is preferred, but new NPs with clinical experience gained as an RN will be considered. You must also be generally computer proficient, and willing to travel in the Albany market. We prefer candidates with experience in long term care and/or managed care. A Master's degree is essential. If you meet these requirements, and you are an energetic and organized individual, you could be the ideal candidate for this position.
